DO I... REGISTER WITH THE PRACTICE?
To register with the practice it will be necessary for you to complete a registration form which is available from reception staff. You will need to live within
the practice catchment area (see map page). Proof of residency will be requested from all patients wishing to join the practice. This can be verified by a recent utility bill, or other
official correspondence eg bank statement, letter from a government agency (Pensions Agency, Home Office, DWP or HM Revenue and Customs). When you want to see a doctor, wherever possible we will try to give you
an appointment with the GP of your choice.

DO I... CHANGE MY PERSONAL DETAILS?
If you change your name, address or telephone number please inform the receptionist as soon as possible. This helps us keep your records up to date and ensures recall
appointments are sent to the correct address. If you move outside the practice area it will be necessary for you to find a doctor in your new area.
