OBTAIN A REPEAT PRESCRIPTION?
Some patients may need long-term medication for which a consultation is not needed on every occasion. In such cases your doctor can issue a repeat prescription. To improve the service to our patients, repeat prescriptions are now computerised. Your doctor will have entered the details of your medication onto the computer and when a new prescription is printed a form is also issued for your next request. Please deliver or post your repeat prescription form to the surgery and allow 48 hours for it to be processed. We regret we are not able to accept requests for repeat prescriptions over the telephone. If you are unable to collect your prescription from the surgery the receptionist can arrange for it to be collected and issued by a designated chemist. Alternatively, we can post your prescription to you. Please enclose an SAE when you make the request. Please do not leave your request until you have nearly run out of your medication - allow at least one week before.

OBTAIN A SICKNESS CERTIFICATE?
A “self-certificate” is used for the first six working days (ie one week including Sunday) of any illness. The form SC1 is available from the
Health Centre, your employer or the Post Office. Whilst we do not normally issue doctors' certificates for this period, a private certificate can be obtained for a fee. After the first week certificates are
obtained from your doctor.
